dosing errors come from people reconstituting two vials differently and forgetting which is which
Reconstituted at 2mL when I meant 2mL. Caught it because the units looked wrong. Write the concentration.

Disagree but this is the good kind of wrong — it is specific enough to be checked.
Slight fix: the number was 99.1, not 93.7. Decimal point, but a fairly consequential one.
