20mg vial, 2mL water, concentration is 20/2 — write it on the vial afterwards

The single most useful thing I did was write the concentration on the vial. Stopped all the confusion after that.

Two vials, two different concentrations, forgot which was which, almost made a 19 times error. Now I label everything.

Yeah, showing the arithmetic is the fastest way for someone to spot the error.

comment on anyone else notice concentration kicking in around week 40 in c/reconstitution · 2 points · 5 months ago

bacteriostatic and sterile water are not interchangeable, do not use them that way

Cosigning. The concentration on the vial in marker is load-bearing.

dosing errors come from people reconstituting two vials differently and forgetting which is which

No. This is the kind of confident post that gets copied into a screenshot and repeated for years. Where is the evidence.

comment on bacteriostatic water vs units — genuinely asking which matters more in c/reconstitution · 43 points · 1 year ago

the concentration on the vial in marker is the most important step

never post a calculation as advice without stating the concentration
