the food noise question that gets asked weekly, answered properly. Making the case below, and I expect to lose some of it in the comments.
The escalation schedule in the trials existed to keep people on the drug. It is a tolerability ramp, and it is why the label reads the way it does.
Steady state after a dose change takes four to five half-lives, so the fair judgement point on a new step is about a month, not four days.
The weekly interval comes out of the half-life — roughly a week — so you are on a smooth curve rather than a series of spikes. That is also why moving your day slightly does very little.
If somebody has the same thing measured a different way, post it next to mine and we will see whether they agree.
best — the order this archive was captured in
Delayed gastric emptying is most pronounced early and attenuates with continued dosing, which is why the constipation usually fades at a stable dose.
This matches me almost exactly, right down to the nausea showing up on the second day and being gone by the fourth.
2.4 is a ceiling, not a target
Weight change and appetite change are different endpoints on different timescales. Conflating them is where most of the confusion on this board starts.
Did protein intake change in the same window?
How long was the stall before you decided it was a stall?
Genuine question — is the appetite effect still there, or is it just the scale that has paused?
the ladder exists because the gut needs the time, not because the pharmacy likes paperwork
What dose and how many weeks at it before this started?
Are you measuring anything other than weight? Waist is usually the one that keeps moving.
Standard reminder: no personal titration plans for other members, and no medical advice. Everything else is fine.
do not chase somebody else’s titration schedule, they are not you
STEP-1 ran 68 weeks and landed just under 15% mean body weight change on 2.4mg. It is an average of a wide distribution, not a target you have missed.
Same injection day each week, or has it drifted?
Does constipation track the day after the shot, or is it there all week?
staying at 0.5mg for an extra month is a legitimate plan
What did week 93 look like compared with this one? The month either side is the useful comparison.
protein first, then everything else gets easier
nausea that arrives on day two and fades by day four is the standard shape
nausea that arrives on day two and fades by day four is the standard shape
Adding one thing to this: the appetite effect and the scale run on separate clocks, so both halves can be true at once.
